☐ I understand that 24 hours’ notice is required for cancellations or rescheduling. Late cancellations may result in loss of deposit.
☐ I can safely use the stairs and get on and off the massage table independently.
☐ I have disclosed all relevant health conditions, injuries, medications, and allergies.
☐ Where necessary, I have consulted my doctor and received clearance for massage.
☐ I understand that massage is not medical care and I am responsible for informing Delila of any health changes.
